How We Help

Civic Heart Community Services is one of the largest, minority-founded social service organizations in the Gulf Coast Region of Texas. For over 35 years the organization has been successful in developing and implementing outreach, education, prevention, intervention, and collaborative partnerships to serve marginalized and vulnerable communities. Civic Heart accomplishes its mission to help empower people to help themselves using a holistic approach.

The organization offers a wide array of program and services that includes: education, workforce development; after-school and summer enrichment programs; housing and supportive services; substance misuse prevention and co-occurring mental disorder treatment; HIV prevention; teen pregnancy prevention; increase access to health care coverage; reentry program and employment; and enhance civic engagement to address challenges and facilitate a greater sense of inclusion.

Our Programs Include

ACA Health Navigator

The ACA Health program provides trained Navigators to help individuals and families gain access to health insurance.

Connecting Kids to Coverage

Connecting Kids to Coverage is an outreach and enrollment initiative to educate families on how they can enroll children in the Children’s Health Insurance Program (CHIP) and Medicaid.

HIV Wellness

The HIV Prevention program uses several methods and interventions that have proved highly effective in reducing the risk of, and protecting against, HIV infection.

HYPE

Helping Youth Prevent Engaging in Risky Behaviors (HYPE) is a teen pregnancy prevention and positive youth development program.

Northern Third Ward

The Northern Third Ward Implementation Project is a twenty-four-member steering committee working to promote and enhance the quality of life in the Third Ward community of Houston, Texas.

Refugee Youth Mentoring

The Refugee Youth Mentoring Program is focused on meeting the unique needs of youth as they adapt to life in a new country.

Rescue in Motion Housing Program

Rescue in Motion helps persons experiencing homelessness with housing, case management, and other essential support services including connection to employment and other resources.

Rescue Youth Substance Use Prevention

The Rescue Youth Program is an evidence-based prevention program delivered in schools, after school and community settings to youth ages 16-18 for the prevention of drugs, alcohol, tobacco, and other risky behaviors.

Teens Making a Choice 2.0

The TMAC program, an evidenced-based teen pregnancy prevention program, teaches skill development in risk avoidance and provides activities that support positive youth development.

VOICES

The VOICES program is a wraparound case management program that works with girls under the age of 18 involved in the justice system.

Youth C.A.N.

The Youth C.A.N. program serves and supports youth under 18 who are placed in juvenile residential facilities in Harris County, Texas.

Youth Justice Community Reinvestment

The Community Reinvestment Fund prioritizes investment in communities with high rates of juvenile justice referrals to prevent crime before it happens by supporting local organizations that engage youth at risk.
