Connecting Kids to Coverage

Civic Heart Community Services reduces the number of children who are eligible for, but not enrolled in, Medicaid and CHIP (Children’s Health Insurance Program), and improves retention of enrolled children.

The target population for the Connecting Kids to Coverage (CKC) program includes children and pregnant individuals living in Harris County and/or Houston, Texas. Civic Heart bridges racial and demographic health coverage disparities by targeting subgroups of children and pregnant individuals with lower-than-average health coverage rates.

Civic Heart staff works in partnership with the State and other stakeholders to:

  • Conduct outreach and education
  • Provide assistance to beneficiaries needing to renew coverage
  • Assist applicants that may have been inappropriately denied benefits and need help to re-apply.
