Houston has made great strides in addressing homelessness, but many individuals with disabilities continue to face barriers to permanent housing and stability. Civic Heart Community Services remains committed to meeting this need through programs that are purpose-driven and system-aligned.
Rescue In Motion (RIM) 2.0 is a permanent supportive housing program designed to serve chronically homeless individuals with disabilities. Operated by Adult Services, RIM connects eligible individuals with long-term housing, intensive case management, rental subsidies, and supportive services to help stabilize their lives and prevent future homelessness.
Important: RIM does not provide emergency rental assistance, shelter, or intake by phone or email.
To be considered for this program, individuals must first go through Houston’s Coordinated Entry system. This system matches people to available housing programs based on their needs and eligibility.

Rescue Youth Substance Use Prevention
The Rescue Youth Program is an evidence-based prevention program delivered in schools, after school and community settings to youth ages 16-18 for the prevention of drugs, alcohol, tobacco, and other risky behaviors.
Self-Sufficiency Fund
The Self-Sufficiency program provides occupational training that leads to an acceptable industry recognized certification for adults who are At-Risk of becoming dependent on public assistance and individuals earning less than $37,000 annually with a dependent child.
